The best strategy is to follow the two closest of the other people on the boat, picking up their puke more often than the more distant guy (whichever that may be).
Every 20 seconds go and clean the puke from the more distant guy as well. When the distant guy becomes one of the two 'paired' guys, follow them again while getting the 'distant' guys puke every so often. You'll get through.
The most annoying part of Mystery Island's boat trip is the cave of terror though; I've been killed by all three. Rarely I can pick the Stone Giant and it'll let me continue as well... Random generators can be annoying, but at least its only a 1 in 3 chance of failing.
